Step 1. Request enrollment in the Vaccine Administration Management System (VAMS) online at https://dphsubmissions.ct.gov/OnlineVaccine
Step 2. Check your email for an email from no-reply@mail.vams.cdc.gov. Click on the link in the email and create your account. You will be prompted to retrieve a code that will be emailed to you. You will need to create a password – store this in a safe place.
Step 3. Complete your VAMS registration.
Step 4. Schedule your appointment at your chosen vaccine clinic. Search “06483” or “Naugatuck Valley Health District” for NVHD’s COVID-19 Vaccine Clinic.
